Our vision at Fempower Physiotherapy is for every person to build a healthy relationship with their pelvis, and bloom into the season of life their body is in — be it a recent diagnosis of endometriosis, recurrent miscarriages, a desire to return to gymnastics and weightlifting postpartum, or anything in between. We plan to pursue that vision by emphasizing the importance of education and evidence-based care so that our clients feel both empowered and informed as they navigate these seasons of life.

While the name may sound like pelvic floor physical therapists only treat the pelvis, the truth is: the pelvis is only a portion of the picture. Pelvic floor physical therapy is actually an all-encompassing service. Our role is to support primarily the musculoskeletal system by addressing tone, strength, and coordination of the muscle throughout the body, assess quality of connective and skin tissue in the midsection and pelvis, then work to rectify any imbalances by improving neuromuscular connection where a connection may be offline or impeded. Treatment often addresses primarily the muscles and connective tissues of the abdomen, back and the pelvis, but often includes muscles in the hips, legs, and upper back, as well.

Pelvic floor symptoms are often a sign that the musculoskeletal and/or nervous system is unbalanced, and some muscles are overcompensating, and others, under-performing. This is a common side effect of major physiological changes, such as chronic gastrointestinal conditions, chronic reproductive conditions, many causes of infertility, pregnancy, and menopause. Over time, unbalances in the musculoskeletal and/or nervous systems can result in tissues that are stressed to the point where that stress has created symptoms. Symptoms often do not come out of nowhere but are more likely a cumulative effect from prolonged imbalances in the system.

Our job is to address your symptoms to improve your comfort and quality of life, then correct those imbalances to the system that contributed to the onset of your symptoms in the first place. Our goal at Fempower is to empower our patients to ensure they not only understand how to relieve their symptoms, but how to independently maintain the progress they make in therapies to prevent symptoms from returning.

What is pelvic floor physical therapy?

What does a standard exam look like?

A standard exam will begin with full-body alignment and movement assessments. This gives your provider a good baseline to grasp the way your body prefers to move, and therefore, which muscles are likely dominating your movement patterns. It is important during this phase to look at the movements that flare your symptoms based on what you share with us — for example, if someone experiences pain while bending over to lift their toddler, we want to see what bending over to pick up their toddler might look like!

The movement assessment is typically followed by an abdominal exam, an external pelvic exam, and an internal pelvic exam. The abdominal exam gives us crucial information about the strength, tone, and coordination of the pelvic floor, as the muscles of the abdomen and pelvic floor work in tandem. The external exam gives us good information about health of the tissue, and the internal exam confirms all previous and suspected findings and allows us to introduce our most effective treatment methods.

Please note: the internal pelvic exam is not required, and all of our patients have the autonomy to opt out if they so choose.

What does treatment entail?

Treatment often includes some combination of the following interventions:

  • A variety of hands-on manual therapies

  • Strength training and therapeutic exercise

  • Movement and coordination retraining

  • Cupping

  • Movement assessment and weightlifting assessment

  • Dry needling, external

  • Bladder, bowel, intake or output diaries

  • Patient education

  • Home exercise program education

  • Exercise program modification for your movement and lifestyle preferences
